GuildCare Pharmacy of the Month – May 2016 – LiveLife Pharmacy Noosaville

The busy Noosaville Live Life pharmacy is situated in the heart of Noosa. We provide a host of health services to our customers, the majority of whom are of an aging demographic.

Feedback we receive from these patients is that they appreciate and benefit from working closely with their health care professionals. We know our patients want to have more input into how they take their medication and also what medication they take on a day-to-day basis.

To facilitate this, we have a designated optimal care pharmacist who spends time on the pharmacy floor, providing an opportunity to discuss treatment options, diabetes health checks, wound care services and also performs MedsCheck and Compliance programs.

GuildCare “on-screen prompts” are fundamental to our ability to assess which patients are eligible for these services. Undertaking compliance checks with our patients gives them a sense of accountability and an incentive to improve their health outcomes through understanding the importance of compliance.

Customers who support MedsCheck program benefit from one-on-one time with our optimal care pharmacist, which enhances their health literacy. We also build on our inter-professional relationships by incorporating/referring to our naturopath and other allied health practitioners in the local area.

The programs that GuildCare offer complement our optimal care pharmacist model thus enhancing patient outcomes and enabling us to provide a holistic approach to health care.
